Foundations in Primary Care Practice

Module Aims       

This module aims to provide students with the opportunity to develop in-depth awareness and understanding of medicines quality improvement processes and information management in primary care general practice. This module will allow students to develop their skills in undertaking medicines reconciliations and clinical medication reviews and in managing repeat prescribing. In addition, students will develop their knowledge and understanding of the processes involved in the implementation of local and national guidelines and making formulary recommendations.

Outline Syllabus   

Introduction into the primary care environment and how GP practices function (culture, systems and processes, roles, funding)
1. Access to patient information- introduction to IT software
2. Access clinical conditions
3. Navigate a consultation history
4. Find prescriptions (acute and repeat)
5. Find blood results
6. Find communications e.g. hospital letters
7. Use the systems to communicate with colleagues (generally and in an emergency)
8. Interpretation of medicines data
9. Repeat prescribing
10. Medicines reconciliation
11. Medication review for patients with long term conditions
12. Implementation of guidelines and formulary recommendations
13. Clinical audit
