Pathways to wellness

With our 12-week programme of physiotherapy, occupational therapy, and workplace support, we're supporting NHS and social care staff in Bradford.

Our 12-week evidence-based programme helps NHS and social care staff who are experiencing health-related challenges that affect their ability to work.

Who we support

We welcome referrals for NHS employees in Bradford with:

  • Musculoskeletal conditions (back pain, neck pain, upper/lower limb problems),
  • Mild mental health conditions (work-related stress, anxiety, low mood),
  • Mild physical health conditions impacting work capacity,
  • Long-term health conditions causing frequent short-term absence,
  • Persistent pain affecting work participation.

Our Approach

Our multi-disciplinary teaching clinic recognises that health extends beyond the workplace, addressing how physical and mental wellbeing impacts all aspects of daily life. We offer:

  • Comprehensive assessment of physical and psychological factors,
  • Personalised exercise prescription and rehabilitation,
  • Occupational therapy assessment and interventions,
  • Interactive workshops on pain and stress management,
  • Ergonomics and body mechanics training,
  • Support with employer communication and workplace adjustments,
  • Self-management strategies for long-term conditions,
  • Graduated return-to-work planning and implementation.

Am I eligible?

You may be eligible if you:

  • Are currently employed by the NHS and Social Care Service in Bradford
  • Are either off work for less than 3 months OR in work but experiencing frequent health-related absences
  • Are aged 16 or over (16-18 must be accompanied by a parent/guardian)
  • Can attend regular in-person appointments over the 12-week programme
  • Are motivated to engage with the return-to-work process
  • Are willing to participate in both exercise and educational components
  • Have an employer willing to support your participation

How to Access the Service

Complete our initial application form and outcome questionnaire.

Alternatively, referrals can be made through:

  • Self-referral (ages 16-18 not eligible for self-referral)
  • Line manager referral (with your consent)
  • GP or other healthcare professional
  • Occupational health
