The School of Dementia Studies is working with Wolverhampton City Council to conduct DCM evaluations across 5 care homes within the region.

Aims

The aim of the work is to use DCM as a way of assisting care teams to develop their services for people with dementia.

Design

DCM is used in cycles of practice development.

Process

Each care home receives a DCM Evaluation and receive feedback on the mapping.

Action plans are then developed for practice development. Homes are then re mapped after six months to review progress and implementation of actions plans.

Outcome

A full report will be sent to Wolverhampton City Council of the findings from the consultancy
